Piedmont Cotillions, LLC - Liability Release

Below is the liability release that will accompany every enrollment to Piedmont Cotillions and acknowledgement is required.

I acknowledge the contagious nature of COVID-19 and its variants and voluntarily assume the risk that my family, including child(ren), and I may be exposed to or infected by the virus that causes COVID-19 or its variants while onsite or while participating in Piedmont Cotillions, LLC activities and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death.  I understand that the risk of becoming exposed to or infected by the virus that causes COVID-19 or its variants while participating onsite or in class may result from the actions, omissions, or negligence of myself and others, including but not limited to Piedmont Cotillions. LLC ownership, facility employees or management or other Piedmont Cotillions, LLC participants.

I voluntarily agree to assume all of the forgoing risks and accept sole responsibility for any injury to my child(ren) or myself (including but not limited to personal injury, disability, and death), illness, damage, loss, claim, liability or expenses of any kind that I, my family and my child(ren) may experience or incur in connection with my child(ren)’s attendance in Piedmont Cotillions classes or the venue location. 

On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless Piedmont Cotillions, LLC and its owners, venues, and its employees of and from the claims, including all liabilities, claims, actions, damages, cost or expenses of any kind arising out of or relating thereto.  I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Piedmont Cotillions LLC ownership, venues, and employees, whether a COVID-19 infection occurs before, during, or after participating in cotillion classes and activities.

Neither Party will be liable for any failure or delay in performing an obligation under this Agreement that is due to any of the following causes (which events and/or circumstances are hereinafter referred to as “Force Majeure”), to the extent beyond its reasonable control: acts of God, accident, riots, war, terrorist act, epidemic, pandemic, quarantine, civil commotion, breakdown of communication facilities, breakdown of web host, breakdown of internet service provider, natural catastrophes, governmental acts or omissions or changes in laws or regulations.

Piedmont Cotillions, LLC will not be responsible for any illnesses incurred at any of our classes, including exposure to COVID-19 or its variants.
